Replacement Card:

Community Training Associates LLC understands that sometimes people lose their certifications or your card becomes lost, or gets stolen. We have a solution. You can now have an “original” replacement card sent to you by filling out the below form.

You must have taken your course with us. If you did not, please contact the training center where you took your original class. There is a replacement fee that must be paid prior to the replacement being processed.
